Controlled Glucose – All adults 18 and older need to have their glucose screening with fasting labs within the past 12 months. If you are a Diabetic, you must also have a A1c of less than 7 to complete this square.
Controlled Cholesterol – To complete this square you must have a fasting lipid panel with LDL at goal of less than 130 within the past 12 months. If you are a Diabetic or a Heart patient, your goal for the same time period is less than 100.
TDAP Vaccine – Your TDAP vaccine must be documented as administrated in a CVMG office or you must have proof from an outside facility within the past 5 years.
Colonoscopy – All adults 50 + are required to have a colonoscopy documented by a GI within the past 3 – 10 years, or have completed an FOBT (Fecal Occult Blood Test) within the past 12 months. If you are <50, this is a FREE square for you!

Pap Smear / PSA >40 – Females ages 21 – 29 must have a pap smear within the past 3 years; Females ages 30 – 65 must have a pap smear within the past 5 years. Males who are 40 – 65 yrs must have PSA lab/blood work in the past year. If you are a male < 40 years, this is a FREE square for you!
